All PTS, LOR release and F1 waiver requests related to
CRC can be emailed to
[email protected]. Please contact your home centre for all other requests.
Please include the following information for all requests:
- Player name
- Position
- Date of Birth
- Division
- Home association
- Last team played for
- Team trying out for
- Full home address and residence since time
For requests, please note the following to ensure proper forms are requested.
F1 Waivers are required for
ALL Orangeville residents at all divisions as Orangeville is not part of the LOR extended boundaries.
LOR Divisions (Brampton and Caledon) U10 - U13 - LOR Release.
LOR Divisions (Brampton and Caledon) U14 - U16 - LOR PTS.
**To attend another OMHA centre a F1 waiver is required**
